Adults ages 18–64: $89 per month. Children with an enrolled adult: $39 per month. Child-only membership: $59 per month. Family maximum: $249 per month. A three-month initial term applies.

A straightforward monthly membership can make routine primary care easier to plan. Membership eligibility is verified before enrollment and is limited to eligible uninsured and self-pay patients.

Included primary-care access
The proposed membership includes scheduled primary-care visits, chronic-condition follow-up, medication-review appointments, virtual follow-ups when clinically appropriate, laboratory-result review, and referral coordination.

- Blood pressure, diabetes, thyroid and cholesterol follow-up
- Secure messaging during clinic business hours
- Same-day or next-business-day appointments when available
- Virtual follow-up when clinically appropriate
Services with separate charges
Laboratory testing, vaccines, medications, injections, rapid tests, cultures, imaging, specialty or hospital care, physicals, weight-management and hormone programs, IV therapy, controlled-substance management, and aesthetic services are not included.

Patients covered by an insurance plan contracted with Supra—including Blue Cross Blue Shield, UnitedHealthcare, Cigna, Medicare and Medicaid—may not use the membership for services covered by their plan. Insurance coverage is verified before enrollment. Additional requirements may apply.
